Nursing & Clinical Support

Nursing & Clinical Support ensures that Participants with complex, specialised healthcare needs can access professional clinical services safely within their home or community environment.

As an unregistered NDIS provider, Blooming Life facilitates these supports through an RN-led subcontracting model, engaging independent, registered nursing professionals to deliver clinical services in line with relevant regulatory requirements, professional standards, and Participant care plans.

This service focuses on clinical coordination, health monitoring, and risk-informed care planning, supporting stability, continuity of care, and Participant wellbeing.

All clinical services are delivered by subcontracted Registered Nurses, in collaboration with the Participant’s treating healthcare team, and in accordance with medical advice, care plans, and Participant choice.

Clinical Assessments & Care Planning
  • Coordinating health assessments conducted by subcontracted Registered Nurses

  • Supporting the development and review of complex care plans (e.g. continence, epilepsy, bowel care)

  • Assisting in implementing clinical risk management recommendations provided by nursing professionals

  • Liaising with GPs, hospitals, and discharge teams to support continuity of care

  • Supporting updates to Medication Administration Records (MAR) as directed by clinical professionals

Specialist Training & Staff Delegation
  • Coordinating RN-led training and competency assessment for support workers

  • Supporting formal assessment of staff in relation to participant-specific care tasks

  • Establishing escalation pathways for changes in health status or clinical concerns

  • Supporting staff education in early warning signs, symptom identification, and monitoring

  • Ensuring training and delegation processes align with NDIS quality and safety requirements

Medication Oversight & Complex Administration
  • Coordinating clinical review of complex medication regimes by subcontracted RNs

  • Supporting safe storage, handling, and disposal processes for medications and clinical equipment

  • Assisting in implementation of incident reporting and error-prevention frameworks

  • Supporting periodic clinical review of side effects and medication interactions

  • Coordinating RN guidance for PRN (as-needed) medication protocols

Wound Management & Skin Care Oversight
  • Facilitating wound and skin integrity assessments conducted by subcontracted Registered Nurses

  • Supporting implementation of turning, positioning, and pressure care schedules

  • Coordinating nursing-led management of wound dressings and skin integrity interventions

  • Supporting escalation pathways for early signs of infection or skin breakdown

  • Assisting in sourcing equipment in collaboration with occupational therapists and allied health professionals
