Multidisciplinary Collaboration Support helps bring structure and clarity to a Participant’s support network by improving communication and alignment between the people involved in their care.
This service is delivered through a non-clinical facilitation and coordination model, supporting communication between the Participant, their family, support workers, and external allied health professionals.
This service focuses on practical coordination, communication support, and routine alignment, helping reduce fragmentation across multiple services and ensuring consistency in everyday support delivery.
All supports are provided in alignment with the Participant’s care plans, allied health recommendations, and personal choices, with an emphasis on clarity, consistency, and collaboration.
Care Team Communication Facilitation
Supporting communication flow between family members, support workers, and external providers
Assisting in organising non-clinical care team meetings and coordination discussions
Helping maintain structured communication records to support continuity of information
Assisting families and support networks with scheduling routines, shifts, and appointments
Supporting reduction of miscommunication or conflicting approaches across multiple providers
Service Integration & Routine Alignment
Supporting translation of allied health recommendations into practical daily routines and supports
Assisting support workers to understand updated routines and daily care instructions
Reviewing shift notes and routine feedback to support consistency across daily supports
Supporting identification of routine barriers that may affect participation or engagement
Assisting coordination between families and external providers to maintain consistent approaches
Progress Information & Plan Review Support
Assisting with organisation of non-clinical observation notes and routine summaries
Supporting preparation of information for plan reviews and service discussions
Helping collate practical information about daily routines, participation, and skill development
Supporting families and teams to maintain clear communication about ongoing supports
Ensuring all documentation handling respects participant privacy and confidentiality
Family-Centred Coordination Support
Supporting participants and families to participate in care discussions and decision-making
Assisting families to understand the structure and flow of multiservice support arrangements
Providing practical guidance during periods of service change or transition
Supporting connection to community-based and mainstream supports where appropriate
Helping build family and support network confidence in managing daily routines and supports
