Independent Living Transition Support

Independent Living Transition Support assists Participants to navigate major life transitions, specifically moving from a family home or group home into more autonomous living arrangements.

This service focuses on developing the capacity to manage housing transitions, tenancy obligations, household responsibilities, and community adjustment, with an emphasis on residential stability, personal safety, and long-term independent living.

Support is tailored to each Participant’s living arrangement, functional capacity, personal preferences, and goals, with the aim of building sustainable residential independence over time.

Tenancy Preparedness & Rights
  • Understanding lease agreements, tenancy rights, and renter responsibilities

  • Communicating effectively with landlords, property managers, and housing providers

  • Completing housing applications, references, and supporting documentation

  • Managing rent payments, bonds, and basic tenancy obligations

  • Requesting maintenance and reporting repairs appropriately

Household Setup & Utility Management
  • Setting up essential utilities including electricity, gas, water, and internet

  • Budgeting for rent, bills, groceries, and ongoing household expenses

  • Sourcing and organising furniture, appliances, and household essentials

  • Managing mail, digital bills, and important documents

  • Creating a functional, organised, and comfortable living space

Home Safety & Daily Maintenance
  • Developing cleaning routines for a safe and hygienic home environment

  • Identifying home hazards and establishing emergency plans

  • Learning safe use of household appliances and security systems

  • Accessing and maintaining safety essentials (e.g. smoke alarms, first aid kits)

  • Establishing emergency contacts and maintenance response procedures

Neighbourhood Familiarisation & Community Integration
  • Mapping and exploring the local area to identify essential services

  • Locating transport, shops, medical centres, and pharmacies

  • Developing safe travel routines within the community (day and evening)

  • Connecting with local community hubs, libraries, and recreational spaces

  • Building awareness of local support services and emergency resources
