Hospital Discharge & Transition Support assists Participants to make a safe, structured, and supported return home following a hospital stay or clinical admission.
As an unregistered NDIS provider, Blooming Life facilitates these transition supports by focusing on home readiness, routine re-establishment, and practical coordination, working alongside hospital discharge teams, allied health professionals, and subcontracted Registered Nurses where required to support continuity of care.
This service focuses on discharge preparation, home environment readiness, risk awareness, and routine stabilisation, with the aim of supporting a smooth transition, reducing avoidable complications, and promoting comfort and confidence at home.
All supports are delivered in alignment with hospital discharge instructions, allied health recommendations, and the Participant’s recovery goals, bridging the transition between clinical care settings and home-based living.
Discharge Planning & Care Coordination
Liaising with hospital discharge teams, social workers, and clinical staff prior to discharge
Supporting transfer of care plans, medication updates, and discharge instructions
Coordinating with subcontracted Registered Nurses where required to review complex care needs before return home
Assisting families and support networks to understand transition steps, timelines, and supports required
Supporting review of NDIS funding and service arrangements to enable post-discharge care
Home Readiness & Equipment Coordination
Supporting pre-discharge home checks to ensure safety, accessibility, and practical readiness
Coordinating delivery and setup of assistive equipment and rental items (e.g., hospital beds, hoists, shower chairs)
Assisting with minor environmental adjustments to support safe mobility and reduced hazards
Coordinating essential preparations such as grocery stocking, pharmacy pickups, and home setup tasks
Ensuring home setup aligns with occupational therapy and discharge recommendations
Routine Re-establishment & Daily Support
Supporting the development of gentle, low-fatigue daily routines during early recovery
Assisting with personal care, meals, and household tasks in line with post-discharge guidelines
Providing routine prompts to support re-adjustment to home-based living after hospital care
Supporting emotional reassurance and confidence during the transition period
Monitoring general comfort and fatigue levels and adjusting supports accordingly
Health Monitoring & Appointment Facilitation
Supporting transport coordination and attendance at post-discharge medical and specialist appointments
Assisting support teams to record and report daily observations and wellbeing indicators as required
Establishing escalation pathways to subcontracted Registered Nurses or healthcare professionals if concerns arise
Supporting structured medication routines using charts, reminders, and organised systems
Facilitating communication between home support teams and treating health professionals where appropriate
