Complex Care Coordination

Complex Care Coordination assists Participants with complex health, behavioural, or multi-service support needs to organise, navigate, and stabilise their broader network of supports.

As an unregistered NDIS provider, Blooming Life delivers this service through a facilitation and coordination model, supporting communication, organisation, and service alignment across informal supports, mainstream services, and external registered providers where applicable, without acting in a clinical or regulated coordination role.

This service focuses on practical systems support, service mapping, and communication facilitation, helping Participants and their support networks maintain clarity, consistency, and stability across multiple services.

All supports are tailored to the Participant’s care network, family structure, lifestyle needs, and goals, with an emphasis on reducing fragmentation and improving day-to-day coordination of supports.

Multidisciplinary Team Collaboration
  • Facilitating communication between family members, support workers, and external providers

  • Supporting organisation of non-clinical care team discussions and coordination check-ins

  • Assisting in translating allied health recommendations into practical daily support instructions

  • Maintaining structured communication notes and service updates for clarity across teams

  • Supporting Participants and families to confidently participate in care discussions and planning conversations

Care Plan Implementation & Routine Alignment
  • Supporting practical implementation of daily routines and support plans

  • Assisting in identifying gaps, overlaps, or inconsistencies in support delivery

  • Reviewing support worker notes to help identify routine stability and consistency issues

  • Observing environmental or situational factors that may affect daily functioning

  • Supporting Participants to track progress against personal goals and daily routines

Service Access & Resource Navigation
  • Researching and mapping community, mainstream, and informal support options

  • Supporting connection to appropriate non-clinical and community-based services

  • Assisting with onboarding logistics for new support workers or service providers

  • Helping Participants and families understand budget use and service utilisation (where applicable)

  • Identifying inclusive community opportunities to support broader social participation

Routine Stability & Contingency Support
  • Supporting development of backup routines and contingency planning for daily supports

  • Assisting in preparation of non-clinical emergency contact lists and escalation pathways

  • Helping establish structured responses for unexpected changes in support availability

  • Reviewing previous disruptions to support more consistent future routines

  • Providing practical reassurance and coordination support during periods of change or uncertainty
