Community Access & Social Participation
Community Access & Social Participation supports Participants to engage meaningfully in community life when disability-related barriers impact their ability to access, participate, or remain connected to social and recreational activities.
This service focuses on reducing isolation, building community confidence, and supporting regular participation in activities that align with the Participant’s interests, goals, and daily routines.
Support is delivered in a person-centred, relationship-based, and goal-oriented way, tailored to the Participant’s individual preferences, functional capacity, and support needs.
